I don't dispense too much nutritional advice, but I'll sometimes mention that I've had clients clear up from taking probiotics, or that some experience rosacea flare-ups in reaction to wheat. Are there sensitive ways to recommend acne treatments and products? Edward: During the pre-service consultation, I have clients tell me exactly what they want to improve about their skin—sometimes they cite things I didn't notice! Afterward, I'm honest with them; I'll say, 'I notice acne scars, or enlarged pores, or sun damage, etc.' Epley: I wait until I've built a solid relationship with a client before I suggest the intensive acne challenge. Dahlgren: With the many young ladies we treat, it's a sensitive subject. We're very friendly and accommodating throughout the consultation and service, and we welcome their parents into the treatment room. There's a lot more talk involved in the teen facial than any other spa service; we want to make sure they're comfortable and that they understand home care. At what point do you direct severely acneic clients to seek medical care? Edward: If the cause of the acne seems hormonal— when this is the case, there is often also facial hair growth and irregular periods—I refer those clients to an endocrinologist. Or, if the severity of a client's acne is out of my scope—more than 20 pimples, or nodular lesions—she's a candidate for serious medical treatment. I refer those guests to any of several dermatologists with whom I work, who offer lower dosages of serious prescription drugs. Epley: A lot of my clients have already seen doctors, but they don't want to be on prescriptions, so they try my program. If that still doesn't help, I refer them to a dermatologist. Dahlgren: If a teenager has acne that we cannot treat at the spa, we confidentially advise her parents to seek a dermatologist.
